Our Practice
Appointments
We see patients by appointment, unless there is a situation that requires emergency care. In this event, it will be determined if you need to go directly to an emergency room or if we can accommodate your needs in the office on the same day. Please give a 24 hour cancellation notice if you are unable to keep your scheduled appointment.
If you have a managed care or HMO plan, please acquire a referral authorization from your primary care physician for a new or return office visit. If an authorization is not on file at the time of the office visit, we will have to reschedule your visit or collect payment in full for services rendered.
Hours of Operation
Monday, Wednesday, and Thursday, 8:00 AM to 5:00 PM
Tuesday and Friday, 9:00 AM to 5:00 PM
Office hours in Ludington are on 2 Mondays a month from 9:00 AM to 4:00 PM
Payment Information
We will be happy to work with you and your insurance company. However, if your insurance company does not pay, or pays only a part of the total claim, you may be responsible for the balance, including amounts considered to be above your insurance carriers usual and customary fee schedule. Any balances, co-payments, or deductibles are due at the time that service is rendered.
We participate with Medicare, Medicaid and Blue Cross. This means we will bill on your behalf. We participate with many managed care plans. Please check with your provider’s handbook for verification of our participation with your plan.
Payments may be made by cash, check, Visa, Mastercard or Discover.
